Summary:

The purpose of this program is to support states to implement, enhance, or expand the provision of assistance through school-based entities under Medicaid and the Children’s Health Insurance Program (CHIP). This program is intended to bolster state and local infrastructure for the provision, billing, and claiming of Medicaid and CHIP school-based services (SBS) programs.

Funding will support infrastructure development in one of the following three categories, consistent with the current status of the applicant state’s SBS program, as evidenced by the state plan, time study implementation plan (TSIP), and any other relevant Medicaid-approved materials:

  • Implementation of SBS: Funding will support states that have yet to expand the coverage of, and billing for, Medicaid and CHIP services provided in schools beyond that which is provided pursuant to a student’s individualized education program (IEP)/individualized family service plan (IFSP). Funds may be used to help resolve such issues as building out the school infrastructure for providing health care services, training, and assessment of needs, to allow for the provision, billing, and documentation of SBS for Medicaid- and CHIP-enrolled students.
  • Expansion of SBS: Funding will support states that have expanded the coverage and billing of Medicaid or CHIP services provided in schools beyond what is provided pursuant to a student’s IEP/IFSP. Funds may be used to help resolve such issues as:
    • Expansion of services beyond IEP/IFSPs in the state program
    • Expansion of the number and type of providers for Medicaid or CHIP services provided in schools
    • Expansion of the availability of services provided via telehealth delivery systems.
  • Enhancement of SBS: Funding will support states that have expanded the coverage and billing of Medicaid services provided in schools to include any Medicaid- or CHIP-covered service provided to a Medicaid- or CHIP-enrolled student by a Medicaid- or CHIP-participating provider, regardless of whether that service is provided pursuant to a student’s IEP/IFSP, 504 Plan, or any other Medicaid- or CHIP-enrolled beneficiary. Funds may be used to help resolve such issues as:
    • Increasing availability of mental health services
    • Removal of coordination barriers between schools and providers in the community
    • Creation of school-based health clinics

Priority will be given to projects under the implementation of SBS category.

All projects must include a proposed time frame for an initial planning period, which may be be identified in time frames of 3, 6, 9, or 12 months. The planning period should be dedicated specifically for the planning of activities intended to implement, expand, or enhance the SBS program.

All projects must also include partnerships with state education agencies (SEAs). Projects should also include partnerships with education services agencies (ESAs); local education agencies (LEAs) and schools, including charter schools; public boards of education; and other public authorities within the state, as appropriate, to determine the needs and processes to be addressed through the project as well as to implement proposed strategies at the state and local levels.

In addition to providing monetary awards, this program will provide technical assistance through the funding agency's Technical Assistance Center. Award recipients must participate and engage with the funding agency's Technical Assistance Center and disseminate information, in terms of lessons learned, best practices and implementation of SBS, to other schools and LEAs.

Eligible costs include personnel salaries and wages, fringe benefits, travel, equipment, supplies, and consultant/subrecipient/contractual costs.

Eligibility Notes:

Eligible applicants are state Medicaid agencies, which, for the purposes of this program, are defined as the single state agency for medical assistance provided under Title XIX of the Social Security Act. Tribal organizations may be eligible to apply if the organization comes under the auspices of, and will work with, the state Medicaid agency in the implementation of the project.

The state Medicaid agency or designee must be the designated award recipient; however, other entities may apply in conjunction as subrecipients, including any other state agencies, tribes, or tribal organizations providing school-based services (SBS) through the state Medicaid or Children’s Health Insurance Program (CHIP) program.

Eligible applicants may be located in the U.S. territories of American Samoa, Guam, the Northern Mariana Islands, Puerto Rico, and the U.S. Virgin Islands.

No more than one application may be submitted per state Medicaid agency. State Medicaid agencies may receive no more than one award through this program over the duration of the project period.

Faith-based organizations are not eligible to apply.

Applications must be formatted on standard-sized, single-sided, and numbered pages with one-inch margins using at least a 12-point font with an average of 14 characters per inch (CPI). The project narrative must be double-spaced. The project abstract summary, the initial planning period work plan and timeline, the budget narrative, the business assessment of the applicant organization, and any tables in the application may be single-spaced.

The following are required in order to submit an application:

  • Unique Entity Identifier (UEI) number
  • SAM (System for Award Management) registration

Funding Notes:

Up to approximately $50 million is expected to be available to support up to approximately 20 awards of up to approximately $2.5 million cooperative awards through this program.

The total number of awards available through this program will be allocated as follows:

  • At least 10 awards are expected to be allocated to projects under the implementation of SBS category
  • Up to 7 awards are expected to be allocated to projects under the expansion of SBS category
  • Up to 3 awards are expected to be allocated to projects under the enhancement of SBS category

The project period will last 36 months. Award recipients will be funded via three 12-month budget periods, as follows:

  • Budget Period 1: July 1, 2024, to June 30, 2025, for $500,000 of the total award amount
  • Budget Period 2: July 1, 2025, to June 30, 2026, for $1 million of the total award amount
  • Budget Period 3: July 1, 2026, to June 30, 2027, for $1 million of the total award amount

Continued funding will be contingent on the award recipient's satisfactory progress, the award recipient's compliance with the terms and conditions, and the availability of funding.

In general, funds may not be used for the following, unless an exception is specifically authorized by statute:

  • Pre-award costs
  • Matching any other federal funds
  • Providing services, equipment, or supports that are the legal responsibility of another party under federal, state, or tribal law, or under any civil rights laws
  • Providing goods or services not allocable to the approved project
  • Supplanting existing state, local, tribal, or private funding of infrastructure or services, such as staff salaries
  • Satisfying state matching requirements
  • Construction
  • Capital expenditures for improvements to land, buildings, or equipment that materially increase their value or useful life as a direct cost, except with the prior written approval of the funding agency
  • Costs of independent research and development, including their proportionate share of indirect costs
  • Lobbying activities
  • Certain telecommunications and video surveillance equipment, as detailed on pages 41-42 of the NOFA file
  • Meals, except under the circumstances detailed on page 27 of the NOFA file

Funds may not be used for:

  • Providing medical assistance, child health assistance, or other health services
  • Covering services related to school-based services (SBS)
  • Duplicating/supplanting other funding sources that can otherwise be claimed to the Medicaid program under Title XIX of the Social Security Act
  • Covering other Medicaid and Children’s Health Insurance Program (CHIP) services that are otherwise matchable with federal funding under Title XIX and Title XXI of the Social Security Act
